Hearing difficulties can occur for several reasons, including problems affecting the outer ear, middle ear, or inner ear. While some hearing tests measure how softly a person can hear different sounds, other assessments examine how the ear responds to changes in air pressure and sound.

An impedance audiometry test is used to evaluate aspects of middle ear function. It can provide useful information about the eardrum and the movement of the middle ear system, helping hearing professionals investigate certain hearing concerns.

Understanding the purpose of this test can help individuals know what to expect during a hearing evaluation.

What Is an Impedance Audiometry Test?

Impedance audiometry is a group of tests that assess how the middle ear responds to sound and changes in air pressure. Tympanometry is one of the main procedures associated with this assessment.

During tympanometry, a small probe is placed at the entrance of the ear canal to create a seal. The equipment changes the air pressure in the ear canal and measures how the eardrum and middle ear system respond to a test tone.

The results are displayed as a graph called a tympanogram. This graph provides information about middle ear mobility and pressure conditions.

Impedance testing may also include acoustic reflex measurements, which assess the contraction of a small middle ear muscle in response to loud sounds.

Why Is Impedance Audiometry Performed?

The test can help investigate possible middle ear problems that may contribute to hearing difficulties.

A hearing professional may recommend impedance audiometry when someone experiences ear fullness, hearing changes, or symptoms that suggest a middle ear issue. It may also be used as part of a broader hearing evaluation.

The test can provide information that complements pure-tone audiometry and other assessments. However, it does not measure hearing sensitivity in the same way as a pure-tone hearing test, and it cannot independently identify every cause of hearing loss.

How Is the Test Performed?

The procedure is generally straightforward and does not require the patient to respond to spoken words.

Step 1: Ear Examination

The hearing professional may examine the ear canal to check for visible blockage, excessive earwax, or other issues that could affect testing.

Step 2: Probe Placement

A small probe with a soft tip is positioned at the entrance of the ear canal to create a seal. The equipment then delivers a low-level test tone.

Step 3: Pressure Changes

The instrument varies the air pressure within the sealed ear canal while measuring changes in the sound reflected by the eardrum and middle ear system.

Step 4: Recording the Results

The equipment produces a tympanogram showing the ear's response to pressure changes. Depending on the clinical question, acoustic reflex testing may also be performed.

Step 5: Interpretation

The hearing professional interprets the findings alongside the patient's symptoms, ear examination, and other hearing test results.

Is Impedance Audiometry Painful?

Tympanometry is generally quick and non-invasive. Some people notice a temporary feeling of pressure in the ear while the equipment changes the air pressure.

The procedure usually does not involve needles or surgery. However, people with ear pain, recent ear surgery, or other ear-related concerns should inform the professional before the test.

Children may also undergo impedance testing when appropriate. The clinician can explain the procedure and help them remain comfortable during the assessment.

Understanding Tympanometry Results

Tympanometry results are commonly described using different tympanogram patterns.

Type A: Generally indicates a typical pattern of middle ear pressure and mobility under the test conditions.

Type B: A relatively flat pattern that may be associated with conditions such as middle ear fluid, depending on ear canal volume and other clinical findings.

Type C: Generally indicates negative middle ear pressure, which may be associated with Eustachian tube dysfunction.

These patterns are not diagnoses on their own. For example, a flat tympanogram can have different interpretations depending on whether the eardrum is intact and the measured ear canal volume.

A qualified professional must interpret the results in context rather than relying on the graph alone.

Impedance Audiometry vs. PTA Audiometry

Impedance audiometry and pure-tone audiometry serve different purposes.

Pure-tone audiometry, often called PTA, measures hearing sensitivity across different frequencies. The person listens to tones presented at different levels and responds when they hear them.

Impedance audiometry evaluates aspects of middle ear function, including eardrum mobility and pressure-related responses.

Both tests may be recommended during a comprehensive hearing assessment because they provide different types of information.

Conclusion

An impedance audiometry test can help assess middle ear function and investigate possible issues affecting the eardrum or middle ear system. The procedure is generally quick and non-invasive, and its findings can complement other hearing assessments.

If you experience persistent hearing changes, ear fullness, or other ear-related symptoms, a professional evaluation can help determine which tests are appropriate. Understanding the results in the context of your symptoms is an important step toward identifying suitable next steps in hearing care.
