PCOS treatment GandhinagarFinding the right begins with understanding how polycystic ovary syndrome affects hormonal balance, menstrual cycles, metabolism, and reproductive health. PCOS is a common hormonal condition among women of reproductive age, and its symptoms can vary considerably from person to person. Although there is no single permanent cure, appropriate medical care, sustainable lifestyle changes, and regular monitoring can help manage symptoms and reduce potential long-term complications.

What Is Polycystic Ovary Syndrome?

Polycystic ovary syndrome (PCOS) is a hormonal and metabolic disorder associated with irregular ovulation, elevated androgen levels, and, in some cases, a characteristic appearance of the ovaries on ultrasound.

Despite its name, ovarian cysts are not necessary for diagnosis. Some women experience irregular periods or acne, while others struggle with weight changes, unwanted facial hair, or difficulty conceiving.

The condition is also associated with insulin resistance in many patients, which can influence blood sugar regulation and increase the risk of developing type 2 diabetes.

Common Symptoms of PCOS

Recognising the symptoms early can help women seek appropriate evaluation and avoid unnecessary delays in care.

Common signs include:

Irregular menstrual cycles: Periods may occur infrequently, become unpredictable, or stop for several months.
Acne and oily skin: Hormonal fluctuations can contribute to persistent breakouts.
Excess facial or body hair: Elevated androgen levels may cause unwanted hair growth.
Hair thinning: Some women experience scalp hair loss or reduced hair density.
Weight management difficulties: Insulin resistance and other metabolic factors can make weight management more challenging for some individuals.
Fertility concerns: Irregular ovulation can make conception more difficult.

Not every woman experiences all these symptoms. Their severity can also change with age and other health factors.

How Is PCOS Diagnosed?

There is no single test that confirms every case. A healthcare professional generally evaluates menstrual history, physical symptoms, medical background, and relevant investigations.

Depending on the presentation, the assessment may include hormone tests, blood glucose testing, cholesterol measurements, and pelvic ultrasound. Thyroid disorders, elevated prolactin, and other conditions that cause similar symptoms may also need to be excluded.

In adults, diagnosis commonly relies on meeting two of three criteria: irregular ovulation, clinical or biochemical evidence of excess androgens, and polycystic ovarian morphology on ultrasound or an appropriate alternative marker. Other causes must be excluded. Diagnostic criteria differ for adolescents, for whom ultrasound and anti-Müllerian hormone testing are not recommended to establish PCOS.

Treatment Options for Managing PCOS

Treatment should be tailored to the individual's symptoms, health risks, and reproductive goals rather than following a standard approach for everyone.

1. Menstrual Cycle Regulation

For women who do not wish to conceive, clinicians may recommend hormonal contraceptives to regulate periods and manage acne or excess hair growth. When periods are very infrequent, appropriate progestogen treatment may help protect the uterine lining.

The choice depends on medical history, contraindications, and individual preferences.

2. Improving Insulin Sensitivity

Some women benefit from medicines such as metformin, particularly when metabolic concerns or insulin resistance are present. A clinician can determine whether it is appropriate after reviewing individual risk factors and test results.

Medication should not be started without professional guidance.

3. Fertility Support

Women trying to conceive may benefit from treatments that encourage ovulation. Letrozole is generally recommended as the first-line pharmacological option for ovulation induction in many women with PCOS-related anovulatory infertility, when other infertility factors are absent.

A fertility specialist can assess the most suitable approach based on age, medical history, and reproductive goals.

Lifestyle Changes That Support Hormonal Health

Daily habits can make a meaningful difference in managing symptoms and improving metabolic health.

Balanced nutrition: Include vegetables, whole grains, protein sources, healthy fats, and fibre-rich foods.
Regular physical activity: Combine aerobic exercise with strength training according to individual ability.
Consistent sleep: Aim for sufficient, regular sleep to support overall health.
Stress management: Breathing exercises, yoga, counselling, or other relaxing activities may help improve well-being.
Sustainable weight management: For women who are overweight, an individualised approach may improve metabolic markers and menstrual regularity. Weight loss should not be treated as a requirement for receiving care.

No single diet or exercise programme works for everyone, and restrictive diets may be difficult to sustain.

When Should You Seek Medical Advice?

Consult a qualified healthcare professional if periods repeatedly become irregular, excessive hair growth develops, acne persists, or pregnancy is difficult to achieve. Seek prompt assessment for unusually heavy bleeding or prolonged gaps between periods.

Early evaluation can help identify associated conditions, including elevated blood glucose, abnormal cholesterol levels, and endometrial risks linked to prolonged untreated absence of menstruation.

Conclusion

Managing PCOS involves more than addressing individual symptoms. Accurate diagnosis, personalised treatment, healthy routines, and ongoing follow-up can help improve menstrual health, metabolic outcomes, skin concerns, and fertility where relevant. Women experiencing possible symptoms should seek evidence-based medical advice rather than relying on self-diagnosis or unverified remedies. With appropriate support, PCOS can be managed effectively over the long term.
