Under the microscope, stained tissue reveals its secrets—the architecture of glands, the irregularity of nuclei, the invasion of malignant cells into surrounding stroma. These visual patterns, painstakingly learned over decades, form the foundation of cancer diagnosis.

But the human eye, for all its remarkable capability, sees only a fraction of what the tissue contains. The subtle texture of chromatin, the spatial organization of immune cells, the architectural signatures of molecular pathways—these features are present in every slide but remain invisible to even the most experienced pathologist.

Computational pathology is changing that. By applying artificial intelligence to digitised whole-slide images (WSIs), researchers are now extracting information that was previously inaccessible: the presence of specific gene mutations, the likelihood of treatment response, the risk of recurrence, and the molecular subtypes that determine prognosis. AI models, especially deep learning, have consistently achieved high diagnostic accuracy in digital pathology, with pooled sensitivity reaching up to 96.3% and specificity up to 93.3%.

In a landmark 2026 study published in Clinical and Translational Medicine, researchers developed SparseAGE-MTL, a weakly supervised multi-task multiple-instance learning model for non-small cell lung cancer (NSCLC). It processes one slide in 2.27 seconds, reducing computational time by more than 99% compared with existing models. It outperforms patch aggregation methods by up to 23% and achieves the highest overall classification performance.
