Magnetic resonance imaging followed in the 1970s, revolutionising soft-tissue visualisation. Today, neuroimaging has become indispensable—transforming stroke care from a passive observation to an active intervention, enabling tumour detection before symptoms arise, and providing objective biomarkers for neurodegenerative diseases.

Yet for the non-radiologist, a brain scan remains daunting. The images appear as abstract greyscale patterns. The reports contain unfamiliar terminology. This guide aims to change that—to empower you with the knowledge to understand what you are seeing, to ask informed questions, and to appreciate the remarkable technology that reveals the mind's physical architecture.

Part I: The Imaging Arsenal — Understanding Each Modality

Computed Tomography (CT): The Workhorse of Emergency Imaging

Computed tomography uses X-rays to create cross-sectional images of the brain. It is the most commonly ordered brain imaging study in emergency settings—rapid, widely available, and excellent for detecting acute hemorrhage, fractures, and large strokes.

How It Works: An X-ray tube rotates around the patient's head, firing beams through the skull. Detectors on the opposite side measure how much radiation passes through. Dense tissues (bone) absorb more radiation and appear white; less dense tissues (air, fluid) appear darker.

Magnetic Resonance Imaging (MRI): The Gold Standard

MRI uses strong magnetic fields and radiofrequency pulses to generate images. It produces exquisitely detailed images of soft tissues—the brain's grey and white matter, cranial nerves, and blood vessels—without ionizing radiation.

How It Works: Hydrogen protons in the body align with the magnetic field. Radiofrequency pulses disrupt this alignment. As protons realign, they emit signals that are detected and reconstructed into images. Different sequences weight the images to highlight specific tissue characteristics.

How to Read a CT Brain — A Systematic Approach

Step 1: Orient Yourself

When viewing a CT or MRI, the right side of the image is the patient's left side. This is because the image is viewed as if looking at the patient from the feet.

Step 2: Check the Windows

Brain Window (width ~80 HU, level ~40 HU): For parenchyma

Bone Window (width ~2000 HU, level ~500 HU): For skull and facial bones

Subdural Window (width ~250 HU, level ~50 HU): For extra-axial collections

How to Read a Brain MRI — A Systematic Approach

Step 1: Review All Sequences
